Study On Improving High Blood Pressure In Older Adults With Multiple Chronic Diseases

Description

Seeking people aged 65 and older with high blood pressure and other chronic diseases for a study to see if blood pressure improvements are better, worse or the same when they measure their blood pressure at home with support from a clinical provider or a clinical pharmacist.

Overview

Participants in this study will measure their blood pressure at home and record the measurements in a diary twice a day for the first week, and then as often as needed. They will also share the measurements with either the normal clinic staff or the clinical pharmacist depending on which group they are in. There are two study visits and three phone visits over the 2-year study.

What we're hoping for

We are studying if blood pressure improves when people aged 65 and older with high blood pressure and other chronic diseases measure their blood pressure at home with support from either a clinical provider or a clinical pharmacist.
